And it's overwhelming when you're learning and you realize, oh my goodness, there's so much to check. You try and remember the oral medicine lectures. They tell you to check every single lymph node under the sun. Jaz's Introduction:And then of course, you remember the occlusion, the very few occlusion lectures you had, and you get the patient to bite together and you don't know what you're doing. You don't know what you're checking for, but you're just checking that the bite exists. And then to try and compute all the things you're seeing and try and put it in some sort of diagnosis and explanation, when you're a student, it's tricky. Which is why I've got Emma, our Protrusive Student on today to talk about routine, just routine, gold standard, daily examinations, something that you will see in general practice in the future. Whilst I am recording with Emma Hutchinson, the student, this is suitable for anyone who wants to learn, any dentist at any stage of career, who just wants to get back to basics, back to foundations. For a, what I call a Real World Examination. The reason I call it a real world examination is because the things that I'll be discussing today and what I'll be sharing video form on the Protrusive app under the student scholarship section is a real world exam. So, for example, a comprehensive exam evaluation could take an hour, 90 minutes. You ask different people. Some people do two hour full examinations. And they have my highest respect, and that's amazing. But that level of service is not for the masses. It's not for the general public. Look, some of my colleagues just get 10 minutes to do an entire examination. So I've had to find somewhat of a halfway house. When the patient feels pain during an ID block. When a matrix does not seal the cavity perfectly. When the shade of your crown doesn’t blend as well as you would like. When the endo you did last year failed. All of these cause us anxiety. All of these are failures in some form, and as Dentists, it eats at us. Let’s admit it: we are perfectionists. Life isn’t about being perfect, but it is all to do with the effort we apply to better ourselves day to day. Failure is a major part of success, but being hung up on our shortcomings will only lead to a downward spiral, which in turn will affect our results and general practice. Jaz’s Introduction:And when you grind your teeth left and right, the canines, for example, are taking a load at that point. And when this is happening the patient's joints are in centric relation. So the condyles are as fully seated position is snug position, and there are no quote unquote interferences or slides. I don't worry if you don't know what those things mean just yet, but some of you may remember seeing this. Maybe you don't understand or internalize what this actually means, but you learn what the perfect occlusion was. But then you started to see patients and you realize very quickly that these naturally dentate patients, your normal patients with the normal teeth. Whether they were 30, 50, or 70, they had all sorts of restorations, or sometimes no restorations. But they had anterior open bites, cross bites, and none of it looked like the bite in the textbook.

Sorry, had to get that out of my chest, but we're back. We're back from AES TMJ conference in Chicago. I also went to Midwinter, and this episode will summarize mine and Mahmoud's trip to somewhere where we had longed to go for so long, the American Equilibration Society. Now, This society likes to be called AES because we've kind of moved away from the whole equilibration thing. Jaz's Introduction:It's way more than equilibration. It is occlusion. It is TMJ is the definitive conference sense that brings together all these occlusal camps. And I tell you guys, I was in heaven. I was in education heaven, and I was also in food coma heaven. So join me in Mahmoud on this, like on the field kind of different podcast, gain a sense of the magic of AES 2024 conference. You'll kind of feel like your presence was there. And of course I'll share at least one key lesson per lecture. As well as some interviews from the amazing Protruserati and mentors that I met. Like, I shook hands with John Kois and Frank Spear within an hour, and I didn't even wash my hands. So how did this trip begin? Well, it began with what we call level one permission. Now I joke about this level one permission whenever we organize a trip like last year, a few years ago Porto, this year we're going Sicily in June for Vertipreps and of course for Chicago, I needed level one permission. So people ask me on Facebook, what is level one permission? Okay, level one permission is when you have to ask your spouse if you can go. I know that sounds really weak, but it's a reality when you're a father of two boys, young boys, I had to absolutely beg Sim to let me go to Chicago. I'm so thankful, me and Mahmoud were both thankful to our wives for letting us go.

I was first exposed to them maybe about, gosh, nine, ten years ago. And this mad scientist type figure came on the scene, Dr. David Clark. I had some good success. And as an associate, I never got to purchase the posterior matrices, although I did do a hands on workshop and I was quite impressed. Are you doing like me? Are you doing cusp by cusp? Or have you discovered another technique? Well, I'm going to start by giving you a warning, Protruserati. This episode may completely change the way you place your posterior composites. I'm talking here bread and butter daily restorative dentistry. Jaz's IntroductionIt might just completely foundationally change it to the core. And of course it's got to be for the better. So the technique that we talk about today, the fast modeling technique or the fast modeling bulk technique, this could be an absolute game changer for you in your practice. I know I have internalized it and thanks to our chat with Ahmed Tadfi today, even now I myself am changing the way I place my posterior composites. Because although I enjoy the cusp to cusp and getting the nice anatomy, it's something I was taught on course many, many years ago, and I've just been doing it, and I'm in a good flow. My wife knows it very well because she sees in me that I am the most anxious. I'm the most worried when I'm running late for something, because for me, when you agree to meet someone or go somewhere for a certain time, it's like a promise that you've made. Jaz’s Introduction:So in my values, not breaking that promise and being punctual is actually really important. Now, interesting thing that I think maybe Jordan Peterson said, or I read somewhere, was that this isn't like a blanket rule. Like, even with me, if I'm running late, I'm not happy. However, there are some things that I value more than other things. So, for example, going to a party. Like, if I'm late to a party, I'm chill. I'm okay. Because parties aren't my highest values. Not anymore, they used to be, but that was a long time ago, right? So they're not my highest value. So if I have to do a few tasks or quick do a few emails before I have to go to a party, for example, and I end up running late for that party, it's okay with me. That can sit okay with me. But if I've agreed to meet a friend at a cafe or some friends at restaurants, then you bet I'll be there on time, maybe even early. Because I value those kinds of encounters and those relationships. Now bringing about dentistry, a long time ago I made an episode called Six Signs That You're A Comprehensive Dentist. And one of the signs was that you're running late a lot because you're trying to give your patient a lot of value. And you talk to them, and you nurture them and you answer all their questions and give them that little bit extra of yourself. And if that sometimes means you're a few minutes late. I think that's worth it. But I have got to terms that in operative dentistry, we can sometimes run late and it's okay. You've got to accept that sometimes you underestimate the situation, especially when we have less experience. For example, that caries cavity, which was way bigger than you anticipated, or that extraction where you didn't actually notice that that mesial root had a bit of a curve on it. So these little things, sometimes we underestimate it. Sometimes what happens is that the air abrasion sand needs changing, and the water needs changing at the same time, and your ID block didn't work,

This one I feel kind of selfish posting this one, but it was interviewed. It's about me today, right? It's my story, my origin, what drives me. But I was interviewed by a very fascinating dental student called Nav. Jaz Introduction:Nav Bhatti And he's an up and coming content creator. I liked his style. I liked his realism. Very real world. I always got time for people like that. He's actually a British citizen studying dentistry history in Slovakia, previously having studied in Egypt and Ukraine. So he's got a really interesting story and we didn't actually get time to unpack it because he came to my practice where I work and he shadowed for a bit and then the plan was to record. But while recording about 25 minutes in, which is why this podcast is as long as is, which is why it's a kind of shorter episode. My wife calls me and at this stage, it kind of sums up the entire sort of month I've had, right? My family has been plagued by illnesses like my baby, Sihaan 10 months had like back to back viruses and when the kids are not sleeping we'll get like 2 -3 hours sleep a night and then unfortunately my eldest, almost 5 years old, he was in hospital for 2 nights on IV drips, severe dehydration, projectile vomiting, you name it, the whole shebang and then it finally got to my wife as well. I was the last man standing, I kind of told the universe and this is crazy, I had Gary Vee say this once, he said, I told the universe I'm too busy to fall sick. Right? And so that's my theory of why I didn't fall sick. I just told the universe, no, not today. But anyway, it's been a really challenging few months and then it kind of got epitomized by what happened while we were recording that my wife called me and she was in bed, she just couldn't move. And she had a baby and I was like, oh my God, I better rush home. And thankfully I live very close to where I work. And I said, Nav, I'm so sorry. I've got to go. And so the reason to share that with you is just to remind you that we're all going to have bad months. Okay?

Click below for full episode transcript: Jaz's Introduction: When I qualified in 2013 from dental school, I had never at that stage done a ceramic onlay or any sort of overlay restoration. In fact, it was full crowns when I was qualifying. A lot of the training that I had was on PFM. So porcelain fused to metal crowns and I look at and reflect on my dentistry and how much of it is adhesive and how much I rely on lithium disilicate onlays and overlays. Jaz’s Introduction:Now, part of the learning process, I learned about immediate dentine sealing, which is what today's episode is all about. And I've been doing this since about 2015. It's something I implemented in my protocol because I went on a Jason Smithson course and I saw that he was teaching it and he recommended it. So me being a good listener, I said, okay, if Jason says do IDS, I did IDS. But over the years, the more lectures I've been to, the more literature I read, the significance and power of immediate dentine sealing really cannot be underplayed. In terms of the improvement in your bonding and the overall success and longevity of restorations just makes a lot of sense, which is why I know you're going to love this episode with Dr. Zahid Sheikh, all about immediate dentine sealing, step by step, every little geeky detail will be covered, but more importantly, whether you've never done IDS before, you will feel confident to do so. And if this is something that you do all the time, you get to understand a bit more of the rationale, the science, and the alternative options, different ways of doing it. Zahid does it differently to how I do it. And so you get to listen to both views. In fact, this episode was full of so much goodness, I had to split it into two episodes. So in this episode, we're focusing on the preparation appointment. How do you actually perform the immediate dentine sealing? And then the part two. will be the fit appointment. You've got your lab work back and now you're going to remove your temporary and then reactivate that IDS, whatever that means. You'll figure out what that means.
