It has a wide range of applications in medical settings, including emergency medicine, procedural sedation, and intensive care. Receptor: • NMDA Receptors: Ketamine is an NMDA receptor antagonist, which means it inhibits the action of glutamate, the brain’s primary excitatory neurotransmitter. This blockade contributes to its anesthetic and analgesic effects. • Opioid Receptors: It also interacts with mu and kappa opioid receptors, which contribute to its analgesic properties. • Muscarinic Receptors: Ketamine inhibits muscarinic receptors, which contributes to some of its sedative and amnesic effects. • Monoaminergic Receptors: Ketamine affects monoaminergic receptors, which might explain some of its antidepressant effects. The syndrome is caused by mutations in the PTCH1 gene, which affects the hedgehog signaling pathway. • Major Criteria for Gorlin-Goltz Syndrome: 1. Multiple Basal Cell Carcinomas (BCCs): These typically appear at a young age. 2. Multiple Odontogenic Keratocysts (OKCs): These jaw cysts are often detected in adolescence or early adulthood. 3. This makes it useful in situations where an increase in blood pressure is needed without stimulating the heart, such as in patients with tachyarrhythmias. 2. The condition can be either acute or chronic and can affect individuals of any age, though it's more common in infants and adults over 40 years of age. Signs & Symptoms: Acute Dacryocystitis - Pain: Often severe and localized to the inner corner of the eye. - Redness: In the affected area. - Swelling: Over the lacrimal sac. - Tearing: Excessive tearing (epiphora). - Discharge: Sometimes a purulent discharge. - Fever: In some cases. Chronic Dacryocystitis - Chronic Tearing: Persistent epiphora. - Discharge: Mucopurulent discharge, especially with pressure over the lacrimal sac. - Minimal Pain: Usually less painful than acute dacryocystitis. - Swelling: Can be present but is typically less pronounced. Causes: Dacryocystitis often occurs when the tear drainage system becomes blocked or narrowed. The blockage can lead to bacterial infection. Common causes include: 1. Nasolacrimal Duct Obstruction: The most common cause, which can be congenital or acquired. 2. Trauma: Injury to the nasal or orbital area. 3. Nasal Issues: Deviated septum, nasal polyps, or sinusitis. 4. Foreign Body: Such as stones (dacryoliths) or other materials blocking the duct. 5. Subciliary Incision: - Location: Just below the eyelashes along the lower eyelid. - Purpose: Commonly used for accessing the lower eyelid and orbital floor, particularly in cosmetic procedures such as lower eyelid blepharoplasty or orbital fracture repairs. - Advantages: Provides good exposure and a cosmetic result with a scar that typically blends well with the natural eyelid crease. - Disadvantages: There is a risk of lower eyelid malposition (like ectropion), especially in older patients due to potential damage to the lower eyelid support structures. 2. Age Group: Approximately two-thirds of AOT cases occur in patients who are under 20 years old. It is more common in females than in males. 2. Location: About two-thirds of the cases involve the anterior region of the jaws, particularly the maxilla (upper jaw) more than the mandible (lower jaw). 3. Association with Teeth: Two-thirds of AOTs are associated with an impacted tooth, often the canine. • Clinical Presentation: AOT usually presents as a painless, slow-growing swelling, causing displacement of adjacent teeth. It may also be discovered incidentally on dental X-rays as it often does not produce symptoms. • Radiographic Features: On dental X-rays, AOT typically appears as a well-defined, radiolucent (dark) lesion surrounding the crown of an unerupted tooth.
