hello there! I'm in the throes of going though the new research that's emerging in the menstrual, maternal & menopausal space, and looking for what's going to change how we help women live well at each of these pivotal life stages. There will be a deep dive and big updates in my online Hormonal Health Masterclass later this summer (you can sign up now and get the updates for free when they go live). Expect info on the influence of menstrual health (including endometriosis) on athletes, lots of new info on postnatal rehab and of course, up to date information on musculoskeletal and pelvic health at menopause To start with, I'm going to use this week's podcast to discuss a new 2024 paper by Stimson et al looking at barriers to strength training at perimenopause - why it matters, what the barriers are, what I think they got right in this paper...and what they missed! I had the pleasure of meeting Emily at the World Endometriosis Congress in Edinburgh last May and I was delighted when she accepted my invitation to come on the podcast to discuss her approach to working with people with Endo We discussed: *what health coaching is (and isn't) and the difference between coaching, mentoring & being a clinician *the role of health coaching in supporting people with endometriosis and how it can be the missing link *Emily's defining question - why don't we do the things we know are good for us? *The many roles pilates can play in helping people with endo live well, from the breath and pelvic floor, to restoring movement and building resilience *how central sensitisation can affect people with endometriosis and using a mind-body strategy that has a focus on breath, awareness and exploring length and strength can help counteract the effects of nociplastic pain we discussed a great paper by Maddern et al (2020) titled 'Pain in Endometriosis' and we also discussed 'Thinking Fast & Slow' by Daniel Kahneman. I first met Krystyna at Pelvicon in 2022 and I'm really happy to share this conversation with you. we talked about: gender inclusive care, getting things right and what to do when you get it wrong asking questions...and respecting the answers understanding what trauma informed care looks like - for your patients but also for you as a caregiver putting all this theory into practice and I talked about how I like to use DNIC as an embodiment practice, to get out of our heads and into our bodies, without triggering a protective response I found this to be a thoughtful & insightful perspective from Krystyna - lots of clinical pearls to use straightaway! Dr Christina Prevett has just finished her PhD in heavy resistance training in older adults and is now doing her post-doc looking at resistance training in pregnancy & postpartum She is committed to building a body of evidence to change the narrative about the 'safety' of strength training - at every age and stage of a woman's life We discuss how to avoid frailty and building up the ceiling of resilience, which starts with challenging clinicians to think beyond 3 x 10 with the yellow theraband ;) Muscle mass, oestrogen, pelvic health, bone health, the gendered environment of gyms, accepting risk-tolerance, challenging the myths & perceptions about what is 'safe' from an evidence based perspective and SO MUCH MORE! I know I'm enthusiastic about every episode but this one...it's one that I hope will change how you think about inpatient pelvic rehab? I first came across Rebeca's work a few year's ago when we were both presenting at the Pelvic Academy's Fall Symposium. In this conversation, we discuss: why aren't ob/gyn surgical procedures given the same prehab and rehab skilled care that total hip replacements or total knee replacements (is it an ortho issue vs an ob/gyn issue? but why are patients having prostatectomies given pre and post surgical rehab, but patients having caesarean sections or hysterectomies are not? Could it be a gender issue too?) the importance of prehab and ERAS (Enhanced Recovery After Surgery) and also: the importance of mental health for pelvic rehab clinicians working in pelvic health, why we need to discuss burnout and boundaries and the importance of inter-disciplinary teamwork I'm very grateful to Rebeca for sharing her experience, insights and passion to change the dynamic. She is a driving force in using movement to help people manage the side effects of cancer treatment using evidence based strategies. Hi everyone! This episode with Dr Noa Goodman explores the concepts of being trauma informed, establishing a therapeutic alliance, why a biopsychosocial approach is central to working with all patients, whether we are talking about pelvic pain, hysterectomies, gender affirming surgeries. we also talk about: *the importance of creating safety for optimal healing *paying more than lip service to psychosocial aspects of care *recognising the role of culture, religion, privilege and intersectionality *working together to establish a treatment plan *regulating our own nervous system before we work with anyone else's *why there's no one 'right answer' to patient care *the importance of collaboration & feedback Noa gives specific strategies for having those conversations, both with ourselves and with the people we are working with, from how to reset our nervous systems, to using the PLISSIT model in patient care. We talk about how implicit biases can affect how we interact with other people...the need for clear explanations and boundaries, watching out for dissociation, and of course, avoiding re-traumatising people who are already traumatised.
