Understanding Male Fertility and Semen Analysis Male fertility plays a crucial role in conception, with male factors contributing to infertility in at least 50% of couples struggling to conceive, and being the sole issue in about 30% of cases. The semen analysis is the primary diagnostic tool for assessing male fertility, examining multiple parameters including sperm concentration, motility (movement), morphology (shape), volume, pH, and liquefaction time. Dr. Lew emphasizes that reference ranges can be confusing - for example, while 15 million sperm per milliliter is considered the lower limit of normal, healthy men often have counts of 100-150 million per milliliter. Importantly, no single semen analysis should be interpreted in isolation, as sperm production takes approximately 70 days and results can be affected by factors from months earlier like fever, illness, or stress. Advanced Testing and Treatment Considerations DNA fragmentation testing provides additional insights into sperm health that standard semen analysis cannot reveal. This testing helps distinguish between environmental factors (which may be modifiable through lifestyle changes, antioxidant therapy, or treating conditions like varicoceles) and genetic factors inherent to how an individual produces sperm. Key considerations include: • Testosterone replacement therapy actually shuts down sperm production entirely, despite low testosterone potentially causing low sperm counts • DNA fragmentation increases miscarriage risk and is associated with poorer embryo quality • Treatment approaches range from lifestyle modifications and intrauterine insemination (IUI) for milder cases to IVF with ICSI (intracytoplasmic sperm injection) for severe male factor infertility • Age affects both partners, with older fathers showing increased DNA fragmentation and associated risks for conditions like autism and schizophrenia in offspring, while delayed reproduction generally increases complexity for both male and female fertility. They discuss how treatment decisions are often individualized based on factors like symptom severity, age, and partner fertility status. The experts express hope that within the next decade, their research will lead to clearer treatment pathways for patients with endometriosis who wish to conceive, as well as better understanding of how to ensure safe pregnancies for those who do.
